What is Hospice Care?

Hospice is a special healthcare option for patients and families who are faced with a terminal illness. A multi-disciplinary team of physician, nurses, hospice aides, social workers, bereavement counselors and volunteers works together to address the physical, social, emotional and spiritual needs of each patient and family. The hospice team provides care to patients in their own home.

Did you know that Hospice can send caring nurses and staff to visit with your loved one in their home or in their place of residence, even if they live in a nursing facility?

We can help you decide what is best for your loved one by doing a complimentary consultation visit.

If you are not local, we can even include you on the visit with a video call. Hospice services can provide care for patients with long term chronic illnesses that are entering their end stages and help with symptoms and providing improved quality of life.

Who is eligible for hospice?
When is it time?

Patients are eligible for hospice care when they have been diagnosed with a terminal illness with a prognosis of 6 months or less. At that time comfort care and symptom management become the primary focus, and curative treatment is no longer the patient’s choice or option.
